All employees/students involved in accidents or incidents while driving university vehicles must file an online accident/incident report within 24 hours.
Filing Vehicle Accident/Incident Report
Accidents and incidents can be reported through our online accident/incident report form, or by scanning the accident QR code in the vehicle or at the Fleet Office door.
- Photographs will need to be attached to the report.
 - There is no option to save the progress in a report and then go back to finish it or make changes after it has been submitted.
 - If the required fields are not completed, you will be unable to submit the report until all required fields are complete.
 - After the report has been successfully submitted, a representative from Fleet Services will contact the department supervisor/representative for next steps.

24-Hour Roadside Assistance
The Roadside Assistance Program is offered for all university vehicles conducting official university business and is available 24 hours a day, 7 days a week. If you are within 75 miles from the main campus, call Quick Silver Tow at (310) 478-1201. If you are over 75 miles from campus, call National Auto Club (NAC) at (800) 600-6065.
Be prepared to provide:
- Vehicle license plate number
 - Vehicle number
 - Vehicle year, make, and model
 - Vehicle's exact location
 - Description of the problem
 - Your name and phone number
